2021 Biennial Affidavits

The 2021 Biennial Affidavits were mailed on April 30, 2021. They must be returned to the Boston Retirement System by July 31, 2021, to prevent suspension of your August 2021 benefits. Due to COVID-19, please DO NOT bring the form to City Hall. The completed affidavit should be mailed in the postage paid envelope that was with the affidavit, or you may email it to us at retirementboard@boston.gov.

When completing the form, you ONLY need to provide the LAST FOUR digits of your Social Security number.

The person receiving the benefit should fill in their information, whether a member or a beneficiary.

If a member or beneficiary has a Power of Attorney (POA), the POA may sign the affidavit. But the POA forms must be on  file at the Boston Retirement System or the form will be rejected.

If the benefit recipient has passed away, there is no need to return the form. Instead, please notify the Board immediately of their passing and mail a copy of their Death Certificate to the Board at 617-635-4311.

State regulations (PERAC Regulations 840 CMR 15.01) require the benefit verification no less frequently than once every two years for each member or beneficiary who receives a

  • pension
  • retirement allowance, or
  • survivor's allowance.
